This pattern converts to any yarn size with corresponding hook.
These instructions include crochet eyes if using blanket yarn.
I prefer to crochet eyes for my plushies, so I'm afraid I'm not certain where the best placement for safety eyes would be. I would suggest between rows 3&4, on either side of your starting point, about 8 stitches apart.
